How to Enter an Aged Care Facility

Wesley can advise you on the level of care that is right for your situation.

An approval to access Wesley Home Care services or Residential Aged Care, (Low and High Care), must be sought from your local Aged Care Assessment Team.  ACATs are located at most public hospitals. You will need to telephone for an appointment and the Assessor will come to your home and complete an assessment to determine your needs.  There is no cost to you and the Assessor will give you the approval which is commonly known as a 3020 form, together with advice as to the right type of care for you.

Retirement Villages

Our Villages include 1, 2 and 3 bedroom units and one bedroom cottages and will enable you to enjoy the benefits of living independently in modern well-appointed accommodation.  You will have the security of being surrounded by friends and being able to enjoy the amenities and social life in our villages.  Our villages offer 'Flexi-Care' - a comprehensive range of domiciliary services to support you and enable you to live as independently as possible.  All units are provided with an emergency response system so you can summons assistance in an emergency 24 hours per day to give you and your family peace of mind.

Our villages offer over 500 units spread over 3 locations.

Low and High Care Facilities

Our Aged Care Facilities cater for older people with a wide range of needs.  We offer a variety of accommodation and we can provide quality residential services for people from a disadvantaged background. We are pleased to be able to offer this kind of service across a number of Sydney locations. All of our facilities have Chaplains, who in addition to taking services for residents, are available to counsel Residents and their families.  All of our facilities are Commonwealth Accredited and staffed by highly trained and motivated personnel, committed to providing a high standard of holistic Christian care.

Assisted Living Facilities also known as Low Care

Our hostels provide care for those holding a current Low Care approval as issued by an Aged Care Assessment Team. Accommodation is provided in individual bed sitting rooms with en suite bathrooms, each unit includes an emergency call system so you can have peace of mind and someone to respond to your call 24 hours per day.

Residents in our hostels are encouraged to maintain their independence for as long as possible, however; as their needs increase, then our low care facilities can provide meals and personal care, such as assistance with showering, dressing, laundry and managing medications. Residents are encouraged to take part in the many and varied activities and outings planned for their enjoyment, such as video nights, carpet bowls, sing-a-longs, gentle exercise programs, current affairs, craft, shopping trips, picnics and bus outings. All our Aged Care facilities have achieved Accreditation by the Commonwealth Dept of Health and Ageing.

Nursing Home or High Care

Our Nursing Home, H C Foreman Lodge at Sylvania offers High Level Care for those who hold a current High Care Assessment. We offer a range of High care services including respite care.  Our Wellness Program has developed a reputation for excellence and provides a range of gentle therapies including exercise, aromatherapy and music.  Our staff are well trained and will provide you with loving nursing care and support.

Respite within a Residential Aged Care Facility 

Respite Care or short term care is available in both Low & High Care facilities for those whose carer may need a break, or those who may need additional care to recover from an illness and return to independent living.  Respite care is approved for 63 days each year and can be accessed by contacting the facility of your choice and making a booking. You may book for 2, 3 or 4 weeks or more or you may use up the maximum nine week period all at once.   The cost is quite reasonable, however, to access Respite care you will need a current respite approval as issued by an Aged Care Assessment Team.  Dept of Veteran Affairs Gold Card pensioners may be entitled to 28 days respite care to be paid by DVA and should enquire with DVA as to their entitlement before making a booking.
