High level support in the home
Extended Aged Care at Home
Extended Aged Care at Home (EACH) is a program that provides clients with high-level support at home. The EACH package can be individually tailored to a client's needs. Packages include but are not restricted to support with:
- Personal care such as bathing and dressing
- Light house work
- Laundry
- Transport to medical appointments
- Meal preparation
- Shopping
- Emotional and social support
- Pastoral care
- Light garden maintenance
- Nursing
- In-Home respite
In addition to the above, Home and Carer Support Services North provide high level support in the home for the specific needs of people with dementia. Service strategies take into account the behaviours of concern which are associated with this condition. They involve dementia specific case management, flexibility and innovation in the delivery of services and access to specialist dementia services and support. How to access EACH
To access an EACH package, a person must be assessed by the Aged Care Assessment Team (ACAT)
as needing high-level support at home.
Anyone can refer an individual to ACAT for assessment, whether it be the individual themselves, a friend, relative, doctor, social worker or advocate.
For more information about ACAT or to obtain contact details, call the Commonwealth Respite and Carelink Centre (CRCC).
Cost
A weekly fee is negotiated with clients. As per the Aged Care Act, fees are capped at 17.5% of the maximum aged care pension.
