Community Aged Care Packages (CACP)
This program is designed to give frail older people the assistance they need to stay in their own homes. Community Aged Care Packages (CACPs) provide a package of care depending on the needs of the client.
CACPs operate a 24hours, 7days a week on-call service.
How to access CACP
To access a CACP package, a person must be assessed by the Aged Care Assessment Team (ACAT) as needing high-level support at home.
Anyone can refer an individual to ACAT for assessment, whether it be the individual themselves, a friend, relative, doctor, social worker or advocate. For more information about ACAT or to obtain contact details, call the Commonwealth Respite and Carelink Centre (CRCC).
Cost
This program is funded by the Commonwealth Government. A weekly fee is negotiated with clients. As per the Aged Care Act, fees are capped at 17.5% of the aged care pension.
