Community LivingThe Community Living Program provides support to people:

  • With an intellectual disability who have mild to moderate support needs.
  • Live independently or in a group type accommodation in the community.
  • Assessed as not requiring more than 15 hours of Home Support per week, as determined by the Community Living Support Assessment.
  • Residing in the Inner West or South West Sydney region excluding Wincarribee.

Service Aims

To Provide flexible support that aims to increase and maintain a persons independence and involvement in the community. This is provided through education and personal skill development.

Support provided will focus on, but not limited to;

Providing support and developing living skills:

  • Washing
  • Safe use of home appliances
  • Shopping
  • Cooking
  • Health and Well Being
  • Personal care
  • Medication

Household:

  • Maintenance
  • Paying Bills
  • Security.
  • Emergency Procedures

Access to liaise with other services/agencies

  • Homecare
  • Department of Housing
  • Community Nurse
  • Meals on Wheels
  • ACCAT assessment (ageing issues)

Individual Service Contract

Systems are in place to ensure each person receiving a service has an Individual Service Contract. 

An Individual Service Contract outlines the support to be provided and the required hours to assist in the promotion of a person’s independence in the community.
