Respite Services"Breakaway Respite Services"

Wesley Mission Disability Respite Services provides pre planned and emergency respite for both children and adults. Breakaway Respite have two locations, being Northmeade and Minto Heights.

  • Breakaway Children's Respite - support people aged between 5-18yrs of age.
  • Breakaway Respite - Minto Heights provides support to people aged 12yrs to whole of life.

Respite is accessed through a referral to the respite coordinator. Applicants must complete a referral form and needs assessment form, which are assessed by the respite service to ensure that you meet the entrance criteria. A waiting list is maintained for future vacancies.

 

Service Aims

Assist in the maintenance of the relationship between the family and their family member with a disability through the provision of regular pre planned respite and emergency/crisis respite care. People are assisted and encouraged to maintain the necessary social and life skills to enable each individual to have a valued role in the community. Individual needs are discussed with the person and family/carer and documented and implemented during the person's respite stay.

Breakaway Philosophy Statement
"Breakaway" supports families / carers of people with a disability by providing regular away-from-home quality care. Our focus is to identify and meet individual needs of people and families / carers of the service within available resources.

We aim to enhance the person's quality of life through education, open communication, advocacy and role modeling. The service adheres to the NSW Disability Services Act (1993) and Standards as well as the policies and procedures of Wesley Mission.

Breakaway Children's Respite  - Northmead

The service is part time and normally operates from Thursday afternoon until Monday morning during school term and Monday morning through to Friday afternoon during school holidays. Arrival for respite is between 3pm and 5pm during school term. Arrival during school holidays is between 10 and 11.3Oam
Children's Respite is a funded respite service in the Cumberland / Prospect Area. 
Eligibility for respite at Breakaway Children's Respite is based on the following criteria:

  • Children aged 5 to 18 years. 
  • Attending School. 
  • Must reside in Cumberland/Prospect area. 

Breakaway Respite - Minto Heights 

The service is a full time service operating between Monday to Sunday, 47 weeks of the year with two close down periods. Breakaway Respite - Minto has weekend respite from Friday evening to Sunday afternoon and week long respite from Sunday evening to Friday afternoon.
Eligibility for respite at Breakaway Respite - Minto Heights is based on the following criteria:

  • People aged 12 years to whole of life
  • Residing in Campbelltown, Camden, Wollondilly, Liverpool, Fairfield or Bankstown areas. 

WHAT HAPPENS WHEN I ARRIVE?

You need to arrange your own transport to respite.

You will need to bring the following items with you;

  • Clothing for length of your stay, (Please clearly mark your clothing with your full first name or initial and your full family name)
  • Toiletries and personal hygiene products.
  • Medication in a webster pack that you will require during your stay. A medication chart signed by your Doctor must also be completed.
  • Medicare and health care card, if applicable

All these items need to be clearly itemised on a clothing / property list upon entry into respite. This will be checked by staff on your arrival.

"Breakaway" provides a service where people with a developmental disability can enjoy a variety of recreational and leisure activities, social and life skills.

Individual needs are discussed with you and documented in each person's Individual Program Plan.

WHAT HAPPENS WHEN I LEAVE?

 You need to arrange your own transport unless otherwise advised by the respite coordinator.

All your personal items will be returned with you when you leave and checked off against the incoming clothing / personal belongings list.

You will be given daily reports for the time you stayed at respite and an evaluation form to complete and return. Your comments will be valued.

A blank clothing / property list will be given to you for your next respite stay.

Staff will be available to discuss information about the visit to respite and answer any questions you may have.
