Wesley Mission's Community Aged Care Services

Wesley Mission Wesley home Care has been providing care to our community since 1991. Wesley Home care is made up of many different programs. The main aim of these programs is to promote independence and to give older people and people with disabilities the choice to remain living in their own homes for as long as possible.

Below are the programs, which Wesley Home Care is currently providing.

Programs

	Home Modification & Maintenance Service The Program providing home modification, maintenance, adjustments and advice on building related matters to assist the frail aged, people with disabilities and their carers to remain safely and comfortably in their own home.
	Commonwealth Carelink Centre Commonwealth Carelink Centres provide an information service to help put older Australians, people with disabilities and those who care for them in touch with a wide range of community, aged care and disability services.
	Community Aged Care Packages (CACP) This Program is designed to assist frail older people to remain at home rather than moving to residential care by providing a ‘package’ of care based on an individuals’ need.
	Respite We also have a Commonwealth Respite Program, which supports the main carer of a disabled person, allowing them a break from their daily routine.
	Charlie Woodward Lodge Charlie Woodward Lodge is a Low Level Residential Aged Care Residential Facility catering to the needs of residents from a previously homeless or insecure housing background.
	Assistance with Care and Housing for the Aged Program. (ACHA) Program is designed to assist the homeless or people in insecure housing to access more appropriate housing and care options to allow them to remain in the community rather than entry into more intensive forms of assistance.
	Pastoral Care Wesley Home Care Pastoral Services comprises a team of people who are committed to meeting the emotional, spiritual and religious needs of our clients, their families, carers and our staff.
