This is an exciting opportunity to join the Regional Assessment Service with Wesley Mission as a Home Support Assessor. This role will see you support older Australians to continue living independently, by considering their needs and matching them with suitable services to meet those needs. This is a community-based role allowing you the opportunity to travel throughout the Northern New South Wales region each day to work collaboratively with clients in their homes.

About the role

As a Home Support Assessor, you will be responsible for: 

  • completing face-to-face Assessments to determine older Australians’ eligibility for support within My Aged Care and the Commonwealth Home Support Programme (CHSP)
  • undertaking client-centred support planning using a wellness and reablement approach
  • liaising with referral sources and community support organisations to assist our clients to obtain the home supports they require
  • ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal.

About you/ Essential criteria

The successful candidate will have

  • experience in a similar role or the broader community sector, preferably aged care and/or allied health background.
  • Certificate IV in Community Aged Care or equivalent
  • must have or willingness to obtain My Aged Care, Home Support Assessor Training (SOA2)
  • strong communication and interpersonal skills, including the ability to form positive, professional relationships with clients, families, carers and service providers
  • ability to work independently
  • high level of computer literacy skills
  • ability to follow guidelines relevant to the position as well as demonstrated problem-solving capacity
  • current Australian Driver’s License and ability to comfortably drive long distances
  • valid National Criminal History Check and Working with Children Check (NSW or equivalent) or willingness to obtain.


Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010. Generous salary packaging and applicable allowances available. A car is provided for travel during work hours.
