How to access home care services
If you’ve already completed a government assessment and want to access home care services, we’d love to hear from you. But don’t worry if this is all new to you, we’re here to help you along the journey.
Follow our 6 easy steps to access home care services:
1. My Aged Care
Everyone’s journey starts with MyAgedCare.
2. Understand what services are available to you
Wesley Mission is an approved provider for home care packages and the Commonwealth Home Support Program.
3. Book an assessment
You will need to be assessed for eligibility to access government funded services. This is done through My Aged Care. The My Aged Care website can explain more about the process.
4. Mention ‘Wesley Mission’
Once you receive approval for the Commonwealth Home Support Program, ask for Wesley Mission to be your preferred provider.
If you have received approval for a home care package, you can contact your local Wesley Home Care branch and we can organise to commence the service for you, as soon as the government has allocated you a package.
5. Sign up to services you need
Once we know your specific needs, we’ll work together to create support services that are right for you. When you’re happy we’ve got it right, you’ll sign a service plan so we can give you the help you need.
