Consumer Directed Care (CDC)

Consumer Directed Care (CDC) is a new way of providing home care that gives individuals more control and choice in the type of services they would like to receive, and how these services are delivered.

As part of our personal care services, Wesley Mission can act as your nominated provider of CDC, which may involve assisting you with:

  • bathing and showering
  • dressing
  • mobility
  • meal preparation
  • eating
  • fitting sensory communication aids. 

How do I access this service?

To access Consumer Directed Care through the Australian Government’s Home Care Packages, you will need to be assessed for eligibility by an Aged Care Assessment Team (ACAT). Once your eligibility has been confirmed, Wesley Mission will discuss the service with you and how much it will cost. We will also conduct a home visit to gain a better idea of your home life and your specific needs.

How much does it cost?

Fees for personal care vary according to the level of support required. The service is subsidised by the Australian Government an Wesley Mission takes into account a person’s ability to pay.
