We provide transport services for safe mobility, nursing services that provide medical care at home and dedicated social support to combat isolation.
In addition, we provide personal care and assist with daily activities around the home. Our In-home Care and Aged Care services
We offer a range of flexible home care services to suit your individual needs, whether you’re seeking government-funded or privately funded support:
- Through Support at Home, we provide in-home care for older Australians who require ongoing assistance to live safely and independently.
- Our Commonwealth Home Support Program (CHSP) services deliver essential help for day-to-day tasks, ideal for those who need minor support to stay active at home.
- To keep your home safe and accessible, we also offer home care modifications such as safety improvements, mobility aids, and minor home repairs.
- We also offer privately funded home care via our tailored fee-for-service options, providing personalised support without the need for an assessment.

After the My Aged Care assessment, one of our care advisers will visit your home to support you to design a care plan that suits your personal needs and budget. You’re in control every step of the way, ensuring your plan reflects your preferences.
Once you’re happy with the personalised care plan and budget, confirm that you’re ready to proceed.

New home care laws – effective November 2025
From November 2025, the Australian Government has introduced a new home care model. If you are already a participant in the Home Care program, you will not need to reapply for care. The Home Care program (HCP) and Short Term Restorative Care (STRC) programs will become classified as Support at Home. However, a review or reassessment may be required to align with the new model.
Other elements that will change include an increase to eight funding level classifications, a quicker funding process for at-home modifications and a quarterly rather than annual budget cycle system. At Wesley Mission, we can assist you in managing these changes without compromising the consistency of your care.

Understanding Support at Home costs and levels
With the implementation of government-funded Support at Home, the previous four HCP levels are being replaced with eight funding levels, bringing in more nuanced degrees of support ranging from minimal to very high.
- Levels 1–2: for largely independent people requiring infrequent support with household tasks, shopping and brief welfare checking.
- Levels 3–5: for individuals requiring a more regular level of service, including transport, personal care and some degree of clinical health requirement.
- Levels 6–8: for those who have high or complex clinical requirements, needing daily care and close case monitoring.

What types of Support at Home packages are available?
There are four main types of Support at Home services:
1 Clinical support services include nursing care, restorative care and health therapies.
2 Independence services provide personal care, carer respite and assistance with activities for social connection.
3 Everyday living services centre around general help around your home and garden, and include the preparation of meals.
4 Short-term support services include providing home modifications and the provision of equipment to help you remain in your home. Restorative care is also included, as is end-of-life support.Who pays for Home Care Packages?
Can I get an interpreter?
If you speak a language other than English, you can call the Translating and Interpreting Services (TIS) on 131 450. They’re available 24 hours a day, 7 days a week and cover more than 100 languages.
Or, you can call the National Contact Centre on 1800 200 422 between 8am and 8pm on weekdays or between 10am and 2pm on Saturdays. They will organise an interpreter through TIS to support our contact centre.

What is a Home Care Package (HCP)?
The Home Care Package (HCP) program, now replaced by the Support at Home program, is a government-subsidised initiative that offers coordinated support services for elderly Australians whose care needs are complex. It's primary aim is to support older individuals to remain living in their homes safely and independently.
What is the Commonwealth Home Support Program (CHSP)?
The Commonwealth Home Support Program (CHSP) will transition to Support at Home, but not before 1 July 2027.
