Wesley Mission is seeking a passionate and experienced Case Manager to join our Intensive Therapeutic Care – Significant Disability (ITC‑SD) team. About the Program
The ITC‑SD program provides therapeutic residential care to children and young people diagnosed with a significant disability in statutory Out‑of‑Home Care. Our team delivers trauma‑informed, person‑centred support with a strong focus on permanency, recovery and life‑skill development.
About the role
As Case Manager, you will deliver high‑quality case work services to children and young people in Intensive Therapeutic Care. You will lead permanency planning, develop and review case plans, and work collaboratively with therapeutic specialists, house managers and support workers.
