  • Part time- 30hours a week- Maternity position until August 2026
  • Based in Taree and Port Macquarie

About the program

Take Charge of Your Life supports young people aged 15+ in future planning and preparing for leaving care. As a wrap around service we work closely with the case management team to ensure young people are supported as they transition into adulthood and leave the Out Of Home Care system. The clients and case management teams that this position supports are located throughout the mid-north coast of NSW – Taree and Port Macquarie, as such the role involved extensive travel. A fully maintained motor vehicle is included as part of the package

 

About the role

The role works alongside a team of case managers who support young people in Out of Home Care. You will undertake Living Skills assessment with young people and support them to set and achieve their goals. The Future Planning Consultant will oversee the development and finalisation of the After Care plans ensuring that young people receive all of their entitlements and have their future needs addressed.

 

Essential criteria

  • Degree in Social Work, Social Welfare, Psychology or related discipline including Youth Work.
  • Experience working with Young people (15-25 years)
  • Ability to communicate effectively and work independently and as part of a team.
  • Demonstrated ability to liaise, network and maintain relationships with key stakeholders in the Take Charge of Your Life program
  • Ability to relate empathetically to the needs of disadvantaged people and those operating in crisis.
  • Ability to think creatively about supporting young people’s needs and implementing living skills programs.
  • Flexible working hours including evenings and some weekends.
  • Current unrestricted Drivers Licence

 

Desirable criteria:

  • Experience working in Out of Home Care particularly with young people transitioning out of care.
  • Experience in teaching or assisting young people to learn Living Skills.
  • Experience in writing After Care plans

 

Remuneration

Salary and employment conditions are in accordance with the SCHADS award, Level 5 pay point 1-3, approx. along with generous salary packaging. The position also includes the use of a fully maintained motor vehicle.
