About the program
Wesley Youth Services (Sydney) supports young people transitioning from care across a range of programs including Take Charge of your life, Supported Independent Living program, Employment consultant program and Specialist After Care.
The Specialist After Care program is a new addition to our suite of programs. Our Specialist After Care program works across the Sydney, Northern Sydney, and Southeastern Sydney Districts.
About the role
We are currently seeking a candidate with experience in Out of Home Care fill this critical role as our team grows. The Aftercare worker works directly with young people 18-25yrs, supporting them to access services in a range of areas including housing, legal support, mental health, general health, education and employment. The Aftercare workers provides mentoring and a group work where appropriate.

Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010 . Level 5 PP1-3.
