There are four main types of Support at Home services:
1 Clinical support services include nursing care, restorative care and health therapies.
2 Independence services provide personal care, carer respite and assistance with activities for social connection.
3 Everyday living services centre around general help around your home and garden, and include the preparation of meals.
4 Short-term support services include providing home modifications and the provision of equipment to help you remain in your home. Restorative care is also included, as is end-of-life support. Mobility equipment and home modifications reduce the risk of falls and accidents for seniors at home.
Yes, you have the right to change your home care provider at any time. The process is straightforward: let your current provider know in writing (check your service agreement for the notice period, usually 2–4 weeks), then contact us and we'll handle the paperwork and ensure there's no gap in your care.
You keep your existing Support at Home funding level, it simply transfers to Wesley Mission. Many of our clients have switched from other providers and found the transition smooth and stress-free. Call us on 1300 086 906 and we'll walk you through it.
If you speak a language other than English, you can call the Translating and Interpreting Services (TIS) on 131 450. They’re available 24 hours a day, 7 days a week and cover more than 100 languages.
Or, you can call the National Contact Centre on 1800 200 422 between 8am and 8pm on weekdays or between 10am and 2pm on Saturdays. They will organise an interpreter through TIS to support our contact centre.
My Aged Care has information on at-home aged care services in a number of languages.
From November 2025, the Australian Government replaced the old Home Care Package (HCP) system with a new model called Support at Home. If you were already receiving home care, you do not need to reapply, your care continues automatically.
Key changes include: eight funding levels (instead of four), faster access to funding for home modifications, and a quarterly rather than annual budget cycle. The Home Care Package (HCP) program, now replaced by the Support at Home program, is a government-subsidised initiative that offers coordinated support services for elderly Australians whose care needs are complex. Its primary aim is to support older individuals to remain living in their homes safely and independently.
CHSP is a government-funded initiative that provides subsidised support for older Australians seeking basic in-home care. This program helps you with daily tasks at home to support your independence. If you have more complex needs, the Support at Home program may be more suitable.
The Commonwealth Home Support Program (CHSP) will transition to Support at Home, but not before 1 July 2027.
