Take Charge of Your Life

Helping young people in care prepare for adulthood

 

Take Charge of Your Life supports young people in foster care to plan for their future, build independence and achieve their goals. From age 15, participants work with a dedicated Future Planning Consultant who provides mentoring, life skills training, education and employment support, future planning and connections to services that help them transition confidently into adulthood.

 

Who it's for:
Young people aged 15–25 currently in Wesley Mission foster care.

Specialist Aftercare

Practical support for young people leaving care

 

Specialist Aftercare provides tailored support to young people transitioning from out-of-home care into adulthood. The program helps participants access housing, education, employment, health services, financial assistance and community connections, ensuring they have the support they need to build a stable and independent future.

 

Who it's for:
Young people leaving statutory care who require additional support as they transition to independence across Sydney, South Eastern Sydney and Northern Sydney.

After Care Support

Ongoing support for former Wesley Mission care leavers

 

After Care Support assists adults who have previously been in a Wesley Mission out-of-home care program. The service can help people access historical records and files, connect with support services, navigate the National Redress Scheme and obtain information about their time in care.

 

Who it's for:
Adults aged 25 years and over who were previously involved in a Wesley Mission out-of-home care program.

Supported Independent Living (SIL)

Supporting young people to live independently

 

Supported Independent Living provides accommodation and personalised support for young people leaving care. The program offers safe housing, case management and practical life-skills development for up to 24 months, helping participants build confidence, avoid homelessness and successfully transition to independent living.

 

Who it's for:
Young people transitioning from out-of-home care who are referred through the NSW Department of Communities and Justice (DCJ).
