Home modifications

When living at home becomes more difficult, we can provide modifications and support that make things a bit easier and safer.

Our friendly staff will work with you or liaise with your Occupational Therapist or Lifestyle Advisor to ensure modifications meet your needs. As with all Wesley Mission services, you can rest assured that our focus remains on respecting your privacy, dignity and freedom of choice.

Eligibility

Wesley Home Modifications may be able to provide assistance if you are a:

  • carer of a frail aged person
  • carer of an adult or child with a disability
  • person with a disability who requires support and assistance
  • person with minimal assistance needs living at home
  • person with more complex needs living at home.

How to access the service?

You can access Wesley Home Modifications through:

  • Commonwealth Home Support Program (CHSP) and Home Care Packages
  • occupational therapists
  • direct referrals.

Commonwealth Home Support Program (CHSP) and Home Care Packages

After completing an ACAT assessment a Wesley Mission Lifestyle advisor will meet with you and/or a loved one to discuss your needs before we’ll arrange a time to complete the required work.

Occupational therapists

If you’re referred by an occupational therapist a Wesley Mission Lifestyle advisor will meet with you and/or a loved one to discuss your needs before we’ll arrange a time to complete the required work.

Direct referrals

You or a family member can contact Wesley Home Modifications directly on 02 9804 5266 to discuss your needs.

How much will it cost?

Fees are based on the total cost of completed work and your level of income, with an appropriate subsidy applied.

The Department of Health and Aged Care have implemented a Standardised Fees Policy for Commonwealth Home Support Program (CHSP) clients to create consistency, fairness and equality.
