Intensive assistance for aged care support

Wesley care finder supports older people who need intensive assistance to access aged care services.

Supported access to quality aged care services for older people

Our Wesley care finder program can guide older people through the process of accessing aged care services. They’ll support with conducting aged care assessments, identifying needs and connecting older people to appropriate aged care support services.

Fully funded through Primary Health Networks, Wesley care finder is free of charge. If you’re applying on behalf of an older person, know that the older person must give consent prior to you sharing their personal information with a care finder service.

To receive Wesley care finder support, an older person must:

  • have no carer or support person who can help them, or who they feel comfortable with
  • have difficulty communicating because of language or literacy problems
  • find it difficult to understand information or make decisions
  • be reluctant to engage with aged care providers or government
  • be in an unsafe situation if they don’t receive services
  • be eligible for government-funded aged care
  • live in an eligible local government area.

Wesley care finder support is available in the following local government areas:

  • Central Coast
  • Cessnock
  • Dungog
  • Greater Newcastle
  • Hawkesbury/Nepean
  • Lake Macquarie
  • Maitland
  • Mid Coast
  • Muswellbrook
  • Port Stephens
  • Singleton
  • Upper Hunter
  • Western Sydney.

How can Wesley care finder help?

Our care finders can work with you and older people to:

  • access My Aged Care and arrange an assessment
  • attend and provide support during the aged care assessment (ACAT)
  • find and shortlist local aged care providers, services and assistance
  • complete forms, explain service agreements and
  • conduct follow up checks
  • connect with additional community services.
